How Do You Report Income Taxes from Self-Employment?

I work two jobs, one at a bookstore, which pretty much gives me all the paper work I need to do my taxes. However, what do I need for income I made from my freelance work? I work doing the post-production for photo-shoots, and I've made about $1500 since March. DO I even need to report that money? Is there a certain amount that income has to be to be worth reporting? ($1500 doesn't seem worth it to me) But if it's recommended, how do I do this?
